Once submitted, please allow up to 48 business hours for our team to review and process your information.
Before scheduling, we do want to kindly share a few important notes so we can make sure your visit is scheduled appropriately:
Dr. Agha does not prescribe controlled substances or sleep medications. These medications should be managed by the appropriate specialist.
For ADHD medications, we require records from your current prescribing provider before scheduling. This includes a recent office note confirming that you are established, stable, and in good standing with your current treatment plan. It is up to the patient to provide this.
If you are needing medication refills, your first visit will be a dedicated new patient appointment so Dr. Agha can take the time to thoroughly review your medical history, medications, and records.
An annual physical is scheduled separately unless you are currently taking no medications and do not have any medical concerns that need to be reviewed.
