Naureen I. Agha, MD FACP

Texas

Effective January 1, 2021, Texas Health and Safety Code, §§481.0755 requires that prescriptions for controlled substances to be issued electronically, except in limited circumstances, or unless a waiver has been granted by the appropriate agency.

Controlled substances

Controlled substances include all Schedule substances. Our office complies with Texas Health codes and the Medical board requirments and makes no exceptions. Dr. Agha DOES NOT routinely provide pain medications for chronic pain and you will be required to see your Pain specialist for this. There will be no exceptions made and we will NOT FILL or REFILL and medications prescribed by your pain doctor or orthopedic or surgeon. It is in your best interest to have ONE physician prescribing these. For new patients please note that we will not be taking over care of Pain medication management. For sleeping pills or ADHD management we generally refer to a a specialist for diagnosis and workup first.

Controlled substances contract

Patients will need to sign our detailed controlled substances contract and no medications will be prescribed without first signing and understanding the form. NO after hour refills, no EARLY refills, no out of state refills will be made. We must be given at least 3 business days notice for refills. All copays and deductibles apply to your visits. Controlled substances cannot be prescribed via telemedicine.

You must keep up with monthly to every 3 month appointments for refills and discussion of how you are doing with the medications. Your Prescription history will be reviewed prior to this with the PMP Texas and pharmacy board per Texas codes. If there are multiple prescribers or early or recent fills your medicine will NOT be filled early. There is no room for discussion here and patients who are rude or argumentative with the staff or doctor will be dismissed from the practice. New patients with ADHD must provide a psychiatrist note with diagnosis and management plan before scheduling. Without this we cannot fill any medicines. If you are a new patient and do not plan on abiding by these rules please schedule with another physician’s office as they may be a better fit .
