We make sure all our new patients feel welcome and taken care of. During your first visit, Dr. Agha will review your medical history. As our relationship grows we will adjust treatments and address any issues you may have. We typically proceed with follow up and labs if needed at a time that is most convenient for you. There is a doctor for every patient and there are patients for every doctor. We look forward to an amicable ,professional and friendly relationship with our patients. We have a small panel and if we do not develop this relationship we encourage patients to look for a doctor or group that may better serve their needs. Frequent no -shows or same day cancellations will result in patient letters.

Patient financial responsibility

HAVING INSURANCE DOES NOT MEAN YOU ARE COVERED.

Many patients do not realize that the contract they signed with their insurance carries a high deductible and that this deductible applies to their visit and must be collected at scheduling . Please do not be upset at our staff for informing you. It is best to contact your insurance as this is a contract signed by insurer and insured and not up to our office. If we are in network with your insurance we cannot charge a lower or self pay rate. A deductible is a fixed amount a patient must pay each year before their health insurance benefits begin to cover the costs. After meeting a deductible, beneficiaries typically pay coinsurance—a certain percentage of costs—for any services that are covered by the plan. They continue to pay the coinsurance until they meet their out-of-pocket maximum for the year.

Financial responsibility is determined first, by the insured, and second by the adult signing the patient in at the time of the appointment.  Please note that both, if different, will be submitted to collection agencies should the financial obligations of appointments not be met in a timely manner.

Charges will be accumulated and routinely filed with your insurance company during the course of treatment. Charges not covered by your insurance, patient co-pays, deductibles, and co-insurance will be your responsibility and are due at the time of service/scheduling.

Cancellation and No Show Fee

We request that you provide at least 24-hour notice of cancellation. You will get text and voice reminders days before your appointment which you can use to cancel your appointment. This will enable the office to offer that time slot to other patients .

Cancellation Fee 

Office appointments, which are cancelled or rescheduled within 24 hours of your appt time are considered No Shows or Late cancel and you will not be rescheduled unless you have paid the $50.01 fee. If you are a new patient and do not show or cancel your appointment on the day of your appointment time you will not be rescheduled and will need to find another physician. We do not overbook and prepare ahead for every appointment.

Patients who do not show for their scheduled appointment and are called “No Show” for two or more appointments during your course as a patient WILL be dismissed from the practice without any exceptions. Letters will be generated from Billing and no exceptions will be made. We are happy to transfer your records to your next Physician.

No shows are extremely stressful to our workflow and practice. We prepare ahead of your appointment and often times get old records, have the doctor review records and labs all before you step into the exam room. We block a generous amount of time for each appointment and NEVER overbook patients. When a patient No Shows this affects our workflow and ultimately trust in our relationship. We sent multiple reminders prior to each visit via text, phone call and/or email. We have all patients sign a No show dismissal policy when joining the practice.

Patients who do not show up for their appointment and who do not call to cancel or reschedule, will be considered a No Show. No Shows are also subject to a $50.01 No Show fee.  Many times we waive this fee per patient request for the first no-show but we cannot waive dismissal from the practice for two or more No shows.

If a patient No Shows for their initial consultation we will NOT be able to schedule them in the future. No exceptions.

Prescription Refill Policy (without a scheduled appointment)

  • If request is made during normal office hours then please give us at least 1 week notice before asking for a refill for a medicine that is about to run out. Dr. Agha will not be refilling medications that a specialist is seeing you for to avoid mixing prescriptions.

  • New prescriptions will not be issued without a visit to determine cause and dose by physician.

  • For all prescriptions patients must be seen at least once every 6 months in order to check in and review medications prior to refill. Some medications like those for Diabetes and Depression need a much closer follow up.

Controlled substances like sleeping pills, narcotics for pain and weight loss medications as well as hormones like Testosterone, Estrogen and Progesterone will NOT be refilled without a visit and need close physician supervision and more frequent appointments. No exceptions. These medicines come with risk and are controlled for a reason. We believe in patient safety first and Dr. Agha abides by all state polices including reviewing the state registry or PDMP prior to every prescription to ensure there is no prescribing by more than 1 provider. If this is a problem then please consider another office for your care. Patients who do not abide by this signed policy will be discharged from the practice. All patients will be signing a prescription drug monitoring contract as required by state law as of 1-1-2021.Failure to do so at an in person appointment will result in no refill.

Dr. Agha DOES NOT refill another physician’s controlled substance prescription. She does not routinely prescribe sleeping pills or narcotics and would be referring you to a specialist if needed who would be prescribing if needed.

Form Completion Policy (without a scheduled appointment)

  • All forms requiring physician signature and medical review - school, daycare and camp physicals, prior authorizations, FMLA, disability or other paperwork will be a fee of $25.00 for all completed forms AND REQUIRE a visit for the doctor to determine what is needed.

After Hours Call Policy

  • All after hours calls for medical advice or results that are non-emergent will be subject to a $25.00 fee that will be billed directly to you and is your responsibility to pay.

Return Check Policy

  • There is a $35.00 charge for return checks added to your original balance. In addition, we may seek all additional legal remedies provided to us under Texas law. Patient Balance Policy

Dr. Naureen I. Agha M.D. PLLC after filing with Insurance companies will mail you a Patient Balance Statement. Payment in full is due upon receipt of this statement. If you have any questions or dispute the balance, it is your responsibility to contact our billing office within 30 days.

  • If you are not able to pay the balance due in full, you must contact our billing office to discuss a payment schedule.

  • Past due accounts may be referred to a collection agency.

Insurance

While the filing of insurance claims is a courtesy that we extend to our patients, it is YOUR responsibility to: While we are contracted with most insurance carriers, PPOs, and Medicare, we always recommend giving your health insurance plan provider a call to confirm your coverage and benefits. The number can be found on your insurance card.

1. Bring your valid and up-to-date proof of insurance coverage and a driver’s license. If you are a new patient, and do not have these cards, we will have to reschedule you.

2. Complete Patient Information Form and check-in via text at each office visit.

3. Notify office of any changes to your insurance.

4. Be familiar with your co-pay and deductibles and be prepared to pay at each visit. We collect prior to a visit with no exceptions. If there is an issue we will reschedule you to another day .

5. Determine if Physician is network providers prior to first visit. We participate in most managed care plans and will file your insurance plan as may be necessary; however, patients are required to pay for their portion of their health plan benefits at the time services are provided. It is not our responsibility to guarantee your network status. This is a contract between you and the insurance company and you have access to a list of all in network providers. Your deductible and co pay always apply if this is required in your insurance contract. It is fraudulent for us not to collect these but know that the contract was signed between the insurer and the insured. If you do not like your policy you have the freedom to change it. We are not in network with Medicaid and cannot see patients with Medicaid or Ambetter or any subsidiaries even as self pay.

Self Payment

For patients without commercial insurance our self-payment fees are $330 for initial visit- 45 minute consultation and review of medical history (Lab fees, imaging, EKG’s, and vaccines are not included). $200 will be collected for all 25 minute follow up visits. This does not include labs or any other testing needed.

This will be collected at the time of scheduling appointments and only after collection will we be able to schedule.

  • We are taking in new Medicare patients until 7-1-2024 !

    . Dr. Agha does not accept Medicaid or TriCare as primary or secondary insurance. No exceptions. If it is found that a patient has either of these going forward you may be discharged as a patient.

    Our office is entirely paperless. All communications are through our portal and email. We want to be sure we are a good fit for your needs.If this will be a problem, please reconsider your request for an appointment. 

We are not in network with HMO plans at this time

No insurance , No problem!

Our self pay rates are $330 for a 45 minute initial consultation. This does not cover labs, ekg, vaccines or any other testing . Follow up fees $200 to secure a 25 minute follow up . This will be collected at the time of scheduling to secure your time with the doctor.

  1. Call our office to schedule a TeleVisit medication review/refill appointment at 972.312.9169 . You will get a text 15 min prior to your visit and you can click the link to enter the waiting room. Please put in whatever vitals you can and answer all form questions.

  2. You can use a mobile phone or your home computer and please check your mic and camera. If we cannot hear or see you Dr. Agha will call you from a blocked number.

  3. Have a credit card number ready for us to keep on file when you schedule. We will charge your usual copay (if using insurance), or self pay pricing starting at $100 to $199
